Not all acne scars are the same

Acne scarring is one of the most nuanced areas of dermatology — because there is no single type of “acne scar,” and therefore no single treatment that works for all presentations. Selecting the right treatment requires accurate identification of the scar type or types present, your skin tone, the depth of the scarring, and whether active inflammation is still occurring.

He identifies the types of scarring involved and designs a treatment approach — or sequence of treatments — based on your individual profile. Getting clear skin is a prerequisite for treating scars: active, uncontrolled acne must first be addressed before scar treatment begins, as ongoing inflammation will continue to create new damage.

Types of acne scars Dr. Zaks treats

Atrophic scars — the most common type, caused by insufficient collagen production during healing. These include rolling scars, boxcar scars, and ice pick scars, which vary in depth and morphology. The appropriate treatment approach depends on which subtype or combination is present, assessed by Dr. Zaks at consultation.

Post-inflammatory erythema (PIE) — the pink or red marks left after inflammatory acne heals in lighter skin tones. These are vascular in nature and respond well to vascular laser treatment (Vbeam).

Post-inflammatory hyperpigmentation (PIH) — dark spots or patches that develop after acne in medium to darker skin tones. PIH is caused by excess melanin and responds to chemical peels, topical agents, and light-based treatments selected specifically for skin of color.

Hypertrophic and keloidal scars — raised scars occurring when excess collagen is produced during healing. Less common with acne but can occur, particularly on the chest and back.

Dr. Zaks uses a range of in-office and combination approaches: Vbeam pulsed dye laser for PIE and redness; salicylic acid chemical peels for surface texture and PIH; dermal fillers for isolated atrophic scars; and PDT for active acne management preceding scar treatment. Yes, in most cases. Active, uncontrolled acne will continue to create new scarring — making scar treatment counterproductive until the acne itself is adequately controlled.
